I have different ways to convey a sense of realism without breaking the site's rules.
When depicting the overall picture of a town getting sacked or a neighbourhood getting thrashed by an out-of-control mob going from door to door, I will write something like "the men were beaten up and tied up, then they were forced to watch as their wives and daughters were raped repeatedly". I was never asked to change anything in any chapter for doing this. What they really don't want is me being too specific with a girl's age.
If I write "the teen daughter screamed her life out" without any mention of age, then I'm fine. I got used to putting a disclaimer that I also use on Literotica. It simply says that in my story, "teenage" refers to characters who are 18-19 years old. Using "teenage girl" in the story doesn't break much in the sense of a realistic feel while it spares the sensitivity of people who think underage sex is evil and that no one should ever read anything depicting it. (They also pick on Stephen King for his underage gangbang in IT.)
I do what I can living with these rules. I personally can't stand censorship and I think Lois feels the same way. If I really want to write unrestrained, then I do it elsewhere where it's allowed.
