It's honestly hard to tell. My actual fantasies are really fueled by humiliation and forced stripping. Those started very early. The non-con aspect is often more of the natural outcome of the humiliating set up. I discovered this community when I was in graduate school.

If we're talking about a story, I think it really depends, especially if we're getting into first person versus third person omniscient versus third person limited. There are different ways to play with how everyone in the scene feels or experiences the situation using narration.

Again, from a narration standpoint, there are different ways to play with this part of a scene. But I do think there needs to be some drama on part of the victim; otherwise, it's just a standard sex scene. I really enjoy the humiliation aspect of the fetish, so there needs to be some signal, , either from their behaviors or their inner monologue, that the victim is experiencing that.

There are a lot of different places, but some of my favorites are secluded cabins, haunted houses (when a supernatural theme is present), schools, offices, back alleys, bedrooms (particular if it's the victim's home), or some dastardly bad guy's hideout.

Almost always soft. I don't like brutal scenes. Punching, blood, slamming heads are all instant turn offs. While I can tolerate it, I honestly don't really like when a gun is present in a scene either. Knives are fun but only strictly for disrobing the damsel. I think of my preferred scenes being the ravishment scenes from romance novels with the drama and humiliation ratched up.

The humiliation, most definitely.

Some overpowering is welcome (though again, nothing overly violent). Knabbing, carrying, tossing on a bed or the floor, pushing up against a wall, forced bondage, hair pulling, or just overwhelming with numbers are all hot.

With respect to realistic, I'd say no. But some struggling is welcome, especially writhing when being overpowered and especially when being taken. Trying to run away is also a theme I dig. I like pursuit scenes as they add to the drama and tension, especially if there is a pay off.
Again, violent or brutal struggle is a turn off.

It can be fun when pulled off with just the right amount, but I'm not into it when it's over the top.

Yes, I find this incredibly hot. To give an example, in the movie Robot Ninja (a so-bad-it's-good type of movie), there is a forced scene and later an attempted forced scene. Both women are verbally objectified before their clothes are torn off and it never fails to turn me on when that happens. It's kind of like dirty talk but turned up in a very politically incorrect way. I also really like when the narration in a story can be used to degrade the victim—so it's not just the characters saying degrading things, it's the narrator as well.

Often, and they can vary between missionary and being taken from behind.

This is an interesting question for me, because while I'm not really preoccupied with a specific size, without getting into a bunch of navel gazing, I am really interested in the cock thought of as a weapon or as something symbolic in a scene. Yes on kidnapping. Long captivity and other themes I like more in terms of a downer ending rather than being the main plot of the story. For example, I like scenes that focus on the initial encounter and then the epilogue suggests the victim is now part of a harem or is being kept as a pet.

In realistic scenarios, no not really.
However, in sci-fi and supernatural instances, it definitely can be! For example, a woman getting raped by an alien species and then instantly getting pregnant with its offspring is a huge turn on! With that said, some people are into "popping" which is not my thing at all.

As I mentioned above, humiliation is really the driving factor for me, and the woman getting forcefully stripped is a must. There are also some other naughty fantasies I have that sometimes mix with non-con themes. Maybe I'll discuss them at some other time.
