I think this has been around about as long as internet porn. There was a site—back in ye olde days of the Internet—where the main selling point was that the guys glasses had a camera hidden in them. Not sure if it was real, but if so, yeah, that puts the technique at at least twenty-five years old.

Back when the genre started they used smaller handheld cameras, prosumer stuff like Panasonic DV100's and Canon X1's.  Over time they transitioned to smaller cameras and once head mounts for GoPro's and phones became common, they've switched to those.  My guess is if both hands are being used they're either using a head mounted camera or a chest mounted camera, depending on the angles preferred.

Below is a link for a cheap little kit that includes both, as well as some other equipment.  Still nowhere near professional grade but usable for the home shooter.
